Our Client

City Healthcare Partnership CIC (CHCP) are a provider of health and care services to people in Hull, the East Riding of Yorkshire and Merseyside. They provide over 50 services in community settings such as palliative care, psychological wellbeing, urgent care and dental services, and have over 1.7m patient contacts each year. 

Project Brief

CHCP wanted to explore the development of a digital-first health service for looked after children and care leavers, aiming to improve access, engagement and outcomes for this cohort. The project focused on assessing market opportunity, understanding service user needs, and defining a clinically-led service model aligned to the evolving digital primary care landscape. It also aimed to establish the financial viability and strategic case required to support informed decision-making.

The Solution

We performed market research to provide CHCP with an up-to-date view of the fast-evolving digital landscape in primary and integrated care, including the most recent NHS England rules and treatment for providers delivering digital services for out of area patients. We explored the total market potential for CHCP as well as the specific needs of target service users to inform the design of the operating model.

In collaboration with CHCP clinical leads, the Baxendale team developed the service vision and granular operating model, to improve access, service user engagement and experience. We factored in health and wellbeing outcomes for a cohort of young people whose living circumstances and experience of statutory services too often pose a barrier to positive outcomes.

Building on the operating model, we developed an overview of the revenue and cost considerations to illustrate the viability of the new service, highlighting any particular risks. We presented an overview of the overarching benefits, risks, considerations and recommendations to the Senior Management Team.

The Results

Following our presentation of the Strategic Outline Case to the SMT, they gave the green light to progress towards full implementation of the service.
